Social Prescriber

I support people who are struggling with physical or mental health issues, and those with Learning difficulties/Neurodiversity to access help with housing, benefits, social isolation, or finding specialist services. We work together to set goals and get the necessary support in place to achieve them.

Health and Wellbeing Team

Health and wellbeing coaches help individuals improve self-management, motivation, and commitment to lifestyle changes. Experts in behaviour change, we work with people to set personalised goals and improve health outcomes.

As coaches we can support with support weight management, improved diet, increasing activity levels, managing long term conditions, menopause support and help with emotional eating.
